IF YOU HAVE STATE CRIMINAL CLEARANCES SKIP TO step #3
STEP 1: Criminal History Clearance, A State Police Background Check: (*Prep time 10-15 minutes)
Have ready: The full name, social security number, and Date of Birth of applicant
1. Go to https://epatch.state.pa.us/Home.jsp
2. Select “New Record Check”
3. Scroll to the bottom of the page and check the box ⬜ under volunteer acknowledgement section, hit “Accept”
4. Enter the demographic data:
I listed Phoenixville Area School District as the volunteer organization and the district phone number as 1-484-927-5000 for the organization phone number.
5. Click “NEXT”
6. Confirm data and click “Proceed”
7. Complete the demographic info on the next page
8. Hit “ENTER THIS REQUEST”
9. If you are completing for another person enter their data here, if not scroll down. The
view Queued records request should say “1”
10. Then scroll down and hit “Request”
11. Then “submit” (It takes a few seconds for the website to generate your form)
12. Then Click the hyperlink for your control number “R#####” on the left hand side of the box. You WILL need this number to complete the volunteer application on the district website, so you may want to jot it down.
13. Then click “certification form”.
Tip: I HIGHLY Recommend you both PRINT and SAVE this form to your desktop . I labeled this one “Criminal History clearance 2019-MyName” as I also have one for my spouse and it is good to know the year it was created as they are good for 5 years. Your control number can be found between the *** you will need this number later.
STEP 2. Child Abuse Clearances: Prep time 15-20 minutes
HAVE READY:
From the Website:
“Before you start, you should have the following information readily available to help you complete your application:
● Addresses where you have previously lived
● Names of all individuals with whom you have lived to include parents, guardians, siblings, spouses, etc.
● Any previous names you have used or have been known by
● Applicants that do not meet the volunteer application criteria or volunteers who have already received a volunteer certification free of charge within the previous 57 months will need to provide either credit/debit card information for an $13.00 application fee or an authorization code from the organization that is asking you to obtain a Pennsylvania Child Abuse History Certification.”
1. Go to https://www.compass.state.pa.us/CWIS/Public/Home
2. Login to the child welfare portal or Click “create individual account”
3. “Next”
4. Enter info to create Keystone username and ID. Username questions (write username and password info down, or put it in your phone for future use-DO THIS NOW or you WILL forget!!)
5. Click “finish”
6. Go to email for temporary password.
7. Copy this password (highlight, right click, “copy”)
8. Log in again (go to link in step 1) and click “access my login”
9. Scroll to bottom of page and hit “continue”
10. Enter user name and “paste” password from email.
11. Choose how you would like to verify your info (email or questions)
12. Select whether you are using a “public or private device”
13. Create a STRONG password (you know...upper and lowercase letters, numbers, a
symbol, a short phrase and perhaps a segment of your genetic code)
14. AGAIN log out, and now back in (link in step 1) with your new and improved personal
password.
15. “Accept” the terms and conditions
16. Click “continue” again
17. Select “create clearance application”
18. Click “begin’
19. Select the first option ⚪ Volunteer ...
20. Under Category select “other” then under agency name select “Phoenixville Area School District”
21. Click “next”
22. Proceed to enter demographic data (tip: we live in Chester County)
23. Enter every address you’ve lived since birth
24. Now list everyone you’ve ever lived with. (tip: there’s no way to enter a deceased
person)
25. Click “no” you have not received a free clearance since 2014.
26. Click “I hereby affirm” ⬜
27. E-sign with your first and last name
28. Click “no” under Applicant payment
29. Await your clearances. Applications should be processed within 14 days.
From the website: “You may obtain a copy of your Pennsylvania Child Abuse History Clearance in two ways. First, when filling out your online application you can indicate that you would like to receive a paper copy of your application on the "Current Address" screen. Otherwise, to obtain a copy of your clearance certificate you may log in to your Child Welfare Account and go to the "PA Child Abuse History Clearance Account" screen. Here you may view submitted and / or processed applications. All processed applications are available to view and print five years or 60 months from the date of when the application was submitted to ChildLine”
And FINALLY, After you recieve your Child Abuse Clearances in the mail.... (tip: Scan this document into your computer or take a pic on your phone, email it to yourself with “child abuse clearances-2019” as the subject, or save it to your file from step 1)
*STEP 3. The school application: Prep time:10-15 minutes if you have your documents handy.
1. Go to https://secure.infosnap.com/family/actionforms.aspx
2. If you were board approved last year, click “yes”, if not click “no”
3. If you want to update your clearances click “yes”.
4. Under reason for Volunteering click “FOCUS”
5. Enter your demographic information
6. Enter the names, schools, and grades of your students
7. Select schools you’ll volunteer in (remember to click the High School if you plan on
decorating).
8. Select Level II clearance.
9. Enter the certificate ID and issue date from your Child Abuse Clearance form. Upload this document.
10. Enter the Control number and date of clearance from your PA State Police criminal history
clearance (see #12 under Step 2) and upload documents. Note: You may opt to mail these documents or drop them off at the district office at

11. Complete the Employee Volunteer Self-Reporting commitment section (read the second
question carefully)
12. Read the mandated reporter form and attest to reading it.
13. Complete the electronic signature.
Step 4: RAPTOR scan
Now...while you’re waiting for your application to receive approval at one of the School Board Meetings, see dates HERE, feel free to stop by the High School security desk (at the main entrance outside the auditorium) to have your driver’s license scanned (RAPTORed). This allows the office to pre-print name tags in advance of the event and saves our volunteers valuable decorating time.
*NOTE: If you access the volunteer application from the District website, it will list Federal clearances as a 3rd document. This involves fingerprinting and is NOT necessary for FOCUS volunteers.
